Thank you for arranging a face-to-face skin consultation at our clinic. During your skin consultation, we can assess your skin and offer advice and treatment options. If a treatment patch test is required, this may also be done at the time (however this cannot be guaranteed*).  If you could complete the following form so that we can confirm your booking:

* A patch test can not be guaranteed for the following reasons: 1. If the time allocated is spent discussing your concern, we require adequate time for a patch test to be carried out safely and efficiently. 2. If you arrive late, there may be insufficient time. 3. If there are any contraindications to health or patch testing on that day, your therapist may advise it be better for your skin to be tested at another time (which financially is more beneficial than having your first treatment with inadequate settings – safety is paramount).
